About the Role
This position is responsible for the direct person-centred employment supports to supported employees that enable them to maintain employment and to maximise their independence, competence and integration in the workplace. It will require consultation with our supported employees regarding decisions that have an impact on their lives and will guide employees to set and achieve their personal goals.
As an Employment Support Worker, you will:
- Adopt a person-centred practice which puts supported employees, their families or guardians at the forefront
- Check in daily with the Employment Coach and Site Supervisor to create daily support plans
- Implement actions as identified in any Individual Support Plan or Positive Behaviour Support Plan, that align with supported employees' needs and goals
- Work alongside supported employees to maintain focus and build on productivity and capabilities
- Document reportable behaviours through FOLIO and notify Production Team Leader and Employment Coach.
- Record clear and concise notes relevant to the supported employee’s progress towards goals and individualized supports provided.
- Carry out all task and functions in accordance with National Disability Industry Standards and Organisational Policies and Procedures to ensure consistent and best practice service delivery
- Maintain appropriate and effective communication with all stakeholders, including leaders, colleagues and management.
This is a Full-Time position based at: Acacia Ridge
About You
To be successful in this role, you will have:
- Certificate IV in Disability or equivalent, or proven industry experience
- Dedication to person centred support principles & individualized support practices
- Demonstrated knowledge and understanding of the National Disability Industry Standards
- Capability to support people to make choices and decisions to achieve their personal goals
- Ability to work both independently and a in team environment, striving towards a common goal
- Ability to apply understanding of ‘duty of care’ to delivering support services and escalate issues when needed
